Every day, struggling families get help from GAFSC. Our diverse staff works with each family to identify their strengths and needs and connect them to community resources. This includes addressing the well-being of parents and children, providing for basic needs and helping the family navigate the complex social services system. Thousands of families have been given a fresh start at life through our programs and services since 1998.

You can help us do this life-changing work by:

  • Volunteer to lend a hand. Support our cause by providing needed help at special events and projects or support a family enrichment program activity.
  • Facilitate a workshop.  If you have a skill or interest (resume writing, journaling, relaxation techniques, interviewing skills, etc.) that you would like to share with our parents, we would love to hear from you.
  • Open up your space.  If you have a meeting room or facility where we can host community meetings, workshops or training, please consider allowing us to utilize your space.

Description

The Family Support Worker, under the supervision of the Family Rehousing and Stabilization Program (FRSP) Supervisor, provides comprehensive case management services to formerly homeless families living in the District of Columbia. S/he will work from a strengths-based, trauma-informed perspective to help families develop skills, maintain housing, and move toward independence.

  • Provides case management services to families including case planning, linkage to employment/education, mental health and other resources; housing assessment and advocacy; budgeting; counseling and family stabilization.
  • Completes home visits to assess for risks, safety, strengths, and needs of children and families and to monitor and address housing conditions.
  • Assists families in creating and adhering to a monthly budget.
  • Provides crisis intervention to individuals and families who have immediate needs.
  • Works closely with families, empowering themto identify their goals and opportunities to improve their circumstances.
  • Advocates for youth and families to receive services to strengthen their support system(s) through linkage with other service providers and organizations.
  • Maintains consistent communications with families’ landlords to ensure rental payments are being made in a timely manner and to resolve and other landlord/tenant issues.
  • Collaborates with all of the service provider’s involved with the family.
  • Compiles case file records in a consistently organized manner; maintain on-going documentation of case history/progress, including specific requirements of FRSP.
  • Maintains all client notes in the Homeless Information Management System (HMIS) and well as the Customer Assessment Tracking and Case History program (CATCH).

Job Expectation/Goals:

  • Client Contacts: FSW must complete one home visit meeting face to face per month along with three other contacts with each family. If unable to connect with the family within two weeks; use the client non-engagement Standard Operation Procedure (SOP) home visit and consult with your supervisor.
  • Intake: Client intake packet and initial home visit must be completed within seven days of assignment, and have documents signed, provide clients in person the Program Rules & Client Handbook; Case Plan set up in all systems (OCTO QuickBase, FRSP Oversight and HMIS), document all attempts to contact client and complete Intake Checklist.
  • Documentation: Progress notes must be documented in OCTO QuickBase within 48 hours of contact; Supporting documentation as needed is in HMIS (medical, mental health, etc.).
  • Reports and Forms: F-SPDAT: Must be completed within 30 days of case assignment; Then 180 days (6 months) from intake; Then every 6 months; additionally, if there is a life-changing event. Client home visit forms are to be completed by the 3rd of the month. FRSP Services Checklist must be updated monthly (at a minimum). Client Rent Report to be completed before the 10th of the month.
  • Detailed Household Budget: The FSW must complete a detailed household budget within 14 days from the date of initial contact.
  • Training: The FSW will register and complete all required training through the Department of Human Services (DHS) upon hire or transfer to the FRSP and again annually. The FSW must forward training certificates to the Human Resource Director and their supervisor. The FSW must complete CPR and First Aid certification and renew every two years.
  • Successful Program Exit: Development of a plan that facilitates successful Program exit and decreases risks of return to homelessness for at least 85% of families who exit the Program to permanent housing within one year of Program exit.

Qualifications: Minimum High School diploma and five years of experience in community-based social services setting or BSW or BA in a related field; and knowledge of Microsoft Office required. Must be able to obtain a clear police clearance, child abuse registry clearance, and FBI clearance and must possess a valid driver’s license.
